Outside In’s Assertive Community Treatment program provides holistic and intensive mental health services in a community setting. Our caring staff members work together to help people become independent and achieve their goals as they experience recovery.

Who is eligible
Individuals ages 18-24 who have been screened into the Multnomah County Homeless Youth Continuum (HYC), meet diagnostic criteria, and who are eligible to be insured under the Oregon Health Plan (OHP), may be eligible to receive our services. To seek a HYC screening, please contact the Janus Youth Access Center.
Janus Youth Access Center
The single point-of-entry for youth ages 16 to 24 who are experiencing homelessness in Multnomah County and seeking services through Portland’s Homeless Youth Continuum, the Access Center is a mobile program that engages with youth every day of the year to determine eligibility, provide immediate crisis support and access to a wide array of community services. Youth seeking services have immediate access to basic needs and safe shelter off the streets. Screenings can be accommodated 365 days of the year between the hours of 9:00 am and 5:00 pm by calling to schedule a time to meet with a Youth Specialist at 503-432-3986.
Once individuals are screened into the HYC, a referral can be made to our program. Referrals can be sent by a service provider or participants may self-refer.

Services Provided
We provide care for young adults with mental health and any substance use concerns. Our services include mental health and substance use counseling, peer support, housing case management, employment and education support, nurse case management, and psychiatric medication management. Our providers work together using an Assertive Community Treatment (ACT) model to support you in taking the next steps to building the life that you want.
